Passport Requirements

To speed-up the processing time remember to: 
  1. fill-out application form completely
  2. submit all original documents
  3. photocopy all necessary documents, prior to going to the Consulate General

First Time Applications


  • Original/certified true copy and photocopy of Birth Certificate from the National Statistics Office in Manila (www.e-census.com.ph to order online), or if not available, substitute the following:
    • Original/Certified true copy of non-availability of birth certificate from the National Statistics Office (NSO) in Manila
    • Original/Certified true copy of baptismal certificate or any other acceptable public documents
    • Joint birth affidavit by two disinterested persons
  • Newly-born should be registered with the Consulate (FA Form No. 40). Present child's original birth certificate, parent's marriage contract, and original/photocopies of parent's passport. ($25.00 Fee)
  • Three identical colored photos (1.77" × 1.37"), recently taken, front view with royal blue background. Please see photo guidelines.
  • For seamen, in addition to items 1-4, must submit valid Seaman's Book, certification/letter from the ship captain regarding present employment, copy of work contract and shore pass.

  • Three (3) identical colored photos 1.77" × 1.37" (3.5 × 4.5 cm) recently taken, front view, with royal blue background. Please see photo guidelines.
  • Proof of Philippine citizenship (green card, INS notice, employment authorization card, visa, etc.)
  • Additional requirement for minors: notarized parental consent to travel signed by both parents
  • Additional requirements for married female individuals who wish to change their maiden names to their married names:
    • Marriage Contract (see guidelines regarding registration of marriage)
    • Divorce Decree (see guidelines regarding registration of marriage)
    • One (1) colored photo 1.77" × 1.37" of husband and wife

    • $20.00 Amendment fee (for change of name requests) in lieu of Passport Booklet Fee

Lost Passport


This is in addition to the requirements for new passports. All those applying for the replacement of lost passports will be interviewed by a Consular Officer. Processing of lost passport is subject to 15-working-day period on top of the regular 6-week processing time.

What to bring 
 Original Birth Certificate fromthe NSO (www.e-census.com.ph to order online) 
 Police report 
 Affidavit of Loss (describing in detail the circumstances of the loss or theft of the passport 
 Any valid photo identification card (driver's license, ARC, etc.) 
 Marriage certificate and Report of Marriage (for married women). (Note: Marriages contracted abroad must first be reported to the Philippine Embassy/Post for recognition.) 
 Any two of the following:
  • Original copy of non-availability of birth certificate from the National Statistics Office (www.e-census.com.ph to oder online)
  • Marriage Certificate from the NSO (www.e-census.com.ph to order online)
  • Original/certified true copy of the baptismal certificate
  • Joint Birth Affidavit by two disinterested persons. (additional requirements for those presenting item a. above)
  • Voter's Affidavit or Voter's ID
  • School Records (e.g. Form 137, Transcript of records, diploma)
  • Proof of service from the Philippine government
  • Government Service Insurance System (GSIS) or Social Security System (SSS) Records
  • Certified true copy of the birth certificates of any of the following: spouse, children, brothers/sister
  • Death Certificate of spouse or parents
  • Seaman's Book
  • Professional Regulatory Commission Identification
  • National Bureau of Investigation (NBI) Clearance
 
 Fees:
  • $90.00 Passport Booklet
  • $10.00 Fax
  • $25.00 Notarization of the Affidavit of Loss (optional, can be notarized outside)
 
What to fill-out 
  • Application Form

What to do 
  • Photocopy all original documents
  • Bring all original and photocopies to the Consulate for your interview
  • After the interview, get note/reminder from the processor on what additional documents you have to submit and what day you have to return for the processing of your passport application
  • Pay $10.00 for fax/communications fee and $25 for notarization
  • After the 15-working-day waiting period, present note to the processor to access record
  • Pay $90.00 fee for passport booklet
  • Passport will be released the after a six-week period

Change of Name by Marriage


Female applicants changing to married names 
  • For a marriage solemnized outside of the Philippines, but within the jurisdiction of the Philippine Consulate General in New York, the following must be submitted
    • original and photocopy of marriage certificate (extended form), and
    • duly accomplished (three sets) Consulate Report of Marriage Contracted Abroad Form. (Note: Registration of Marriage requires the following: extended form marriage contract (What is an extended form marriage contract?), two passport photos of both parties for each set (a total of six photos), proof of citizenship of both parties); and Passport Application Form (if passport is expired) or Passport Amendment Form (if passport is valid)
  • For a marriage solemnized in the Philippines - Original copy of marriage certificate issued or authenticated by the National Statistics Office (www.e-census.com.ph to order online) and Passport Application Form (if passport is expired) or Passport Amendment Form (if passport is valid)
What is an Extended Form? 
  • An extended form marriage certificate/contract looks just like a typical marriage contract/certificate except that it contains specific civil status information regarding number of previous marriage. Church wedding certificates of marriage will not be accepted.
Female applicants changing from married names to maiden names 
  • For widowed applicants - certified true copy and photocopy of death certificate of husband, or court order of presumptive death
  • For annulled marriages - certified true copy and photocopy of annotated or amended Marriage Contract from the NSO or Certificate of Registration and certified true copy of Court Order
  • For divorced applicants (Allowed only in cases where the non-Filipino spouse acted as the plaintiff; and not allowed when both parties are Filipino citizens at the time of the divorce) Original copy of Divorce Decree

Release of Processed Passport


The processed passport will be released to the applicant after six (6) weeks. If the applicant could not claim the passport personally, he/she could designate a representative to claim the passport on his/her behalf, provided the representative presents a valid photo ID card of himself/herself and presents an authorization letter signed by the passport's owner.
